Mission Statement

The Diabetes Education Program reports to the department of Food and Nutrition of Unity Health System. We are dedicated to diabetes self-management education and support of individuals with diabetes. We serve patients from urban and suburban neighborhoods with varied educational, cultural and socio-economic backgrounds. Our goal is to empower patients to take control of their diabetes through education, so that complications, morbidity and hospitalizations may be reduced and quality of life optimized.

Vision Statement:

Our vision as the Diabetes Education Program of Unity Health System is to remain an American Diabetes Association recognized program that is able to provide:

  • Easy access for all our patients throughout our service area.
  • Qualified staff with a sensitivity to patients' individual and family needs.
  • Networking across the continuum of care so that patients receive seamless diabetes education and care between physician, hospital and community.
  • Documented outcomes upon which decisions are made to continuously improve the program.
  • Patients with a feeling of self-efficacy in managing their disease.

Use of Funds Donated:

  • combination TV/VCR for use in classes and community locations
  • patient video and book libraries
  • update on patient video library
  • national conferences and training for staff
  • laptop/power point projector

$$ Received by Chapter 20:

We've received $1000 every year for the last 7 years, plus a special grant ($2,000) that helped purchase the laptop/power point projector.
